The Community Connections service supports adults to continue to live independently in their own home, either on their own or with their family/carers within Ceredigion. Service users receive individualised support which is tailored to their personal circumstances having consulted with them, their families and other professionals who know them.
Individuals receive varying amounts of support dependent on their assessed needs which may include personal care. The support provided can for example be for housing related issues so that tenancies may be maintained, assistance to manage finances and other paperwork effectively as well as access activities within the community in order to develop and maintain social inclusion.
