Flexible community support

In addition to our support for independent living service, we offer a range of flexible support for people living in the community.

Connecting People

Our Connecting People service supports adults to continue to live independently in their own home, either on their own or with other people.

Connecting People focuses on working with the person to build networks and relationships with others that support people to live as independently as possible, with less paid support.

We work in partnership with other community organisations so that people can access learning, volunteering and paid employment opportunities.

Support is flexible, and the amount we provide is shaped by what people want and need.

Home support

A little support at home can make a big difference.

We provide home support for children and adults who live at home with their family.

Support is flexible, and the amount we provide is shaped by what people want and need. We work together with the people we support, their families, friends and other community members to create support plans that work for each person. People have voice, choice and control in everything they do, including how they are supported.

Some examples of home support include:

  • Help around the home; preparing food, snacks, house work and shopping
  • Support with day to day tasks, such as getting ready for work or school
  • Assisting with personal care
  • Help to get out in the community and visit friends
  • Support with making and attending appointments
  • Support after a stay in hospital
